The Importance of Documentation

Documentation can be used as evidence in court or administrative proceedings. There are different types of documentation that you will be keeping. We will briefly review the types, importance of them, and guidelines.

Records/Logs: Records facts and dates; are descriptive of what actually happened, what was seen and/or heard. Should not include opinions or feelings. Is used to record progress over time. Can be used as evidence. A bound notebook is preferred, but a spiral book, loose leaf, or computer document is also acceptable. A composition book is recommended. All records should be kept in a secured, confidential area.

Journal/Diary : For personal/parental use only. Personal feelings may be recorded. This is not a business record.
